Sewanee ID Form

General Student Information Form: Deadline is June 2, 2008
(for Sewanee ID Card and Sewanee Class of 2012 Picture Book)

To receive a Sewanee ID when you arrive to campus you must complete this form, which includes submitting a digital picture (see specification below).  Using this form saves considerable time when you arrive on campus. Before you ask, we do not mail ID cards. If your application is submitted and photo accepted by the due date (June 2, 2008), your ID may be picked up from your Head Resident when you arrive on campus. You will need to present a Valid ID.

Please submit information by June 2, 2008.  Submissions after this date are welcome but may not be included in the print publications.  Late pictures will still be utilized for your Sewanee ID card (if picture meets specifications). You may continue to send in ID photos until the first of August.

Photo submissions must meet the following requirments to be used for your Sewanee Card

  • Face must be clearly visible and facing forward - no profile shots, sunglasses or hats.
  • Photo must be in color - passport type photos are best! Do NOT USE a copy of your passport.
  • Student must be the only person in the picture.
  • There should be a small space between your head and the top of the image, as well as a definite contrast between you and the background.
  • Photograph must have a uniform light-colored background. (Light skin and light hair work best with dark background, inversely dark hair and dark skin works best with light background.)
  • No graduation caps, gowns, or costumes.
  • There can be no borders on the photo.
  • No red eye or flash reflection on glasses or contact lenses.
  • Nothing can be touching the face such as hands, trees or graduation certificate.
  • The image be at least 15 kilobytes (KB) and it must be less than 1 Megabyte (MB).
  • The image must be at least 300x300 pixels.
  • You may only upload .jpg images.
  • Your card will be made available for pickup during your orientation.
  • For copyright purposes, submissions cannot be professionally  taken photographs.

First, please save the picture file as your last name and first and middle initials with no spaces. (For example Hannah Elizabeth Smith would save her picture file as:  SmithHE (Please save your picture in a jpg format.)

Next, use the Browse button to find your picture file.  Again, it should be named your last name and initials (SmithHE).
