Spring Forms/Deadlines

Welcome to Sewanee: The University of the South.  The purpose of this web experience is to guide you through a series of expectations,
introducing you to a variety of people and programs at Sewanee.  Your responses to these forms help Sewanee maintain a strong tradition of meaningful relationships.   Please complete each form on or before the deadline.   We encourage you to complete them before the deadline

Attention transfer and new students: You must complete all forms.

Readmitted students and students returning from abroad: You must complete the health insurance waiver and car/bike registration forms. (look for red print with **)



DEADLINE: January 6, 2011
Students returning from study abroad must complete this form by December 1, 2010**

1)  Complete the Housing Form (Helps us match you with a roommate and place you in a residence hall.)  



DEADLINE: January 14, 2011

2)  Submit Medical Health Forms (Make an appointment with your health provider.  Housing assignments will NOT BE mailed until health forms are complete. Call Health Service for more information at 931.598.1270.)  Please click on the following links to download the two forms you will need to complete prior to your arrival at Sewanee:  Sewanee Medical Record and Sewanee Medical History.

3) To be exempt from participating in the University's health insurance policy, submit the completed Insurance Waiver Form and proof of quality health insurance.  (Information  | Waiver FormFailure to submit proof of health insurance through the waiver process will automatically enroll you in a student health insurance plan through United Healthcare.  You will be charged $515 on your University bill, which is the cost of coverage.**


4)  Begin to research academic classes you want to take this spring.

5)  Complete the foreign language placement exam (You must take a placement exam before you will be allowed to register for classes.  Because foreign language study usually occupies more semesters to complete than other general distribution requirements, it is essential that you not only take a placement test but also select carefully a language which you will study during your first year at Sewanee.)

6)  Sewanee ID Form (This form requires a digital picture in jpg format, saved as your last name, followed by your first and middle initials - for example SmithHE.jpg for Hannah Elizabeth Smith.) 

7)  Read "Eight Keys to Academic Success" in preparation for transfer and new student orientation which begins on Monday, January 17 at 9:00 a.m. 


DEADLINE: January 15, 2011

9) Car and Bike Registration (Form is only needed if you have a car or bike. Please review Sewanee's parking policy.) **


  ARRIVAL DAY: January 16, 2011 

All new students must arrive on January 16, 2011 to unpack and to settle into your new home. Move in begins at 12:00 noon, please no early arrivals.
Orientation begins on Monday, January 17 at 9:00 AM in the Regents' Room in Walsh-Ellett Hall.
